Governance and Risk Control
Your organisation has an AI policy. It describes principles, assigns responsibilities, and satisfies the auditor's checklist. What it cannot do is intervene when a model drifts, produce the decision trail a regulator will ask for, or flag a bias pattern before it reaches a customer. We are often invited in when that distinction has become uncomfortably clear. It is a statement of intent, and under serious scrutiny, the difference is consequential.
The common failure is not bad intent. It is a category error: treating probabilistic systems with the governance assumptions that apply to deterministic software. Deterministic software does what it is told, reliably and repeatedly. A probabilistic system does what the training data and the input distribution suggest, which means its behaviour shifts as data drifts and deployment conditions diverge from the conditions under which it was tested. Policy written for the first kind of system does not govern the second.
Where Governance Breaks Down
Organisations that have moved AI into production typically discover the same sequence of failures, in roughly the same order.
Adoption Without Foundation
The pressure to deploy is real and the competitive anxiety is legitimate. The mistake is treating AI adoption as a tooling decision rather than an organisational transformation. Models are deployed into workflows without the accountability structures, data governance, or risk controls that would make them manageable. The organisation has AI in production. It does not have AI under control.
Governance That Arrives Late
In our experience, the vast majority of governance is retrofitted. A model ships, an incident occurs or a regulator asks a question, and the framework is written in response. By that point, accountability is already blurred: the model has been modified, the original training data is poorly documented, and the team that built it has moved on. We often see that governance written after deployment struggles to prevent what happens next, and it rarely satisfies the regulator or the board.
The Assurance Gap
The hardest question for most AI programmes is not "do we have controls?" but "can we prove they are working?" A governance framework describes the controls that should be in place. Assurance is the continuous inspection layer that verifies they are functioning as designed, detects when model behaviour has drifted from the baseline, and produces the evidence that a regulator or auditor can examine. Without assurance, governance is a claim. We treat assurance as a live engineering discipline, not a reporting exercise.

Governance Architecture
We define accountability at the point where AI decisions affect outcomes: who owns each model, what decisions it is permitted to influence, what the escalation path is when confidence is low, and how regulatory obligations map to specific system behaviours. We work with engineering and risk teams together to ensure those controls are operational, not aspirational.
Audit Trails and Explainability
We build the logging and explainability infrastructure that makes AI decisions inspectable. When a regulator asks why a particular outcome was reached, the answer should be retrievable from the system itself, not reconstructed from memory. We design that infrastructure before deployment, not after the question is asked.
Continuous Assurance
We implement the monitoring and testing pipelines that verify governance controls are functioning over time: bias detection, drift monitoring, robustness testing, and compliance evidence generation. A model that behaved correctly at launch may not behave correctly six months later. Assurance is the discipline that catches the difference before it becomes an incident.
